ABSTRACT

Objective::To observe the efficacy of Wenfei Jianpi decoction on moderate to severe persistent allergic rhinitis (AR) with Lung Qi deficiency and cold syndrome and its effect on substance P (SP) in nasal secretions and peripheral blood proinflammatory factors. Method::One hundred and sixty-six patients were randomly divided into control group and observation group by random number table. Patients in control group got budesonide nasal spray, 2 times/days, and cetirizine hydrochloride tablets, 10 mg/days. And patients in observation group got Wenfei Jianpi decoction for oral, 1 dose/day. A course of treatment was 8 weeks. Before and after treatment, nasal obstruction, itching, sneezing, runny nose, itchy eyes, tears and nasal sign were scored, nasal airway resistance was detected, and rhinoconjunctivits quality of life questionnaire (RQLQ) were scored. Before and after treatment, levels of SP immunoglobulin E (IgE) in peripheral blood, interleukin-4 (IL-4), interleukin-5 (IL-5), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) were detected, and eosinophil (EOS) were calculated. Result::By rank sum test, the clinical efficacy in observation group was better than that in control group (Z=2.073, P<0.05). Before and after treatment, scores of total score of nasal symptoms, ocular symptoms and subjective symptoms and nasal sign score were all lower than those in control group (P<0.05). Except sleep and emotion, scores of other dimensions and the total scores of RQLQ were all lower than those in control group (P<0.05). EOS in observation group was less than that in control group (P<0.05). And nasal resistance declined in observation group (P<0.05), and levels of IgE, SP, IL-4, IL-5, IL-6, TNF-α and nasal resistance were lower than those in control group (P<0.05, P<0.01). Conclusion::Wenfei Jianpi decoction can be used to treat moderate to severe persistent allergic rhinitis by relieving subjective symptoms and signs, improving quality of life, inhibiting inflammatory factors and reducing allergic reaction.

ABSTRACT

OBJECTIVE: To explore the characteristics of CTG(cardiotocography)and discuss pregnancy management of fetal extrasystole.METHODS: It's a retrospective analysis of women who gave birth in May 2011-March 2018 in PekingUniversity Third hospital.A total of 13 cases were diagnosed as fetal extrasystole through echocardiography or M type ultrasound. The characteristics of CTG,delivery weeks, delivery mode, neonatal arrhythmia diagnosis and treatment weresummarized.RESULTS: In 13 cases, except 2 cases with normal CTG,the others showed that normal fetal heart raterhythm and deceleration alternately appeared, but it was different from hypoxia-induced deceleration.Eight of 13 cases underwent cesarean section, and 4 of these cases were with fetal arrhythmia or fetal distress as an indication. All neonates were alive. One case turned into supraventricular tachycardia, which resolved after receiving medicine. For the other cases, the ECGs resolved or returned to normal within two weeks after the birth.CONCLUSION:s The most common type of extrasystole is atrial extrasystole. When premature beats are not transmitted,we call it blocked atrialextrasystole, which causes deceleration. If there is no heart structure abnormality and heart failure, vaginal delivery canbe performed under closely monitoring.

ABSTRACT

OBJECTIVETo investigate the influences of varying lingual traction forces on the space-closing speed in a typodont model.

METHODSForty-two Angle Class I standard typodont models of bimaxillary teeth protrusion were divided into 7 equal groups. Four regions of the model were paired to groups, and in the odd-numbered models, the top left and bottom left regions served as the experimental group and the top right and bottom right regions as the control group; in the even-numbered models, the regions in the model were grouped oppositely. In the experimental group, the space was closed by niti wire extension spring in the buccal ridge combined with lingual elastic traction of 0, 5, 10, 15, 20, 25 and 30 g. In the control group, the space was closed by exclusive niti wire extension spring in the buccal ridge. The space-closing speed were analyzed in all the groups.

RESULTSThe space-closing speed was significantly lower in the control group than in the experimental groups with lingual traction forces of 5, 10, 15, 20 and 25 g (P<0.05), but a traction force of 30 g resulted in a significantly lower speed than that in the control group (P<0.05). The space closing speed was the greatest in the experimental group with a traction force of 15 g (P<0.05).

CONCLUSIONNiti wire extension spring in the buccal ridge combined with lingual elastic traction results in faster space-closing speed than traditional exclusive niti wire extension spring. The speed is the fastest by applying 15 g lingual traction, which is also associated with the lowest slip resistance.

ABSTRACT

OBJECTIVETo investigate the effect of Schisandrin B on mitogen-activated protein kinases (MAPKs) and nuclear factor-KB in rat lungs exposed to silica.

METHODS92 Wistar rats were randomly divided into four groups: Control (20), Silica (30), Sch-B treated 1 group (Sch-B 1) (24) and Sch-B treated 2 group (Sch-B 2)(18). Silicotic animal models received an intratracheal injection of silica. From the first day after model establishment, rats in Sch-B 1 were treated intragastrically with Sch-B at a dose of 80 mg/kg, once daily. From the 8th day after model establishment, rats in Sch-B 2 were treated intragastrically with Sch-B at a dose of 80 mg/kg, once daily. 6 rats in Sch-B 1 were sacrificed on the 3rd, 7th, 14th and 21st days, and 6 rats in Sch-B 2 on the 14th, 21st and 28th days, 4 Control rats, 6 silica rats on the 3rd, 7th, 14th, 21st and 28th days accordingly, lungs were collected. Right lung for protein extraction, the phosphorylation level of extracellular signal-regulated protein kinase, c-Jun NH2-terminal amino kinase and p38 in lungs were detected by Western Blot. Left lung was fixed by neutral formalin. The ratio of nuclear transfer of NF-kappaB was evaluated by immunohistochemistry.

RESULTS1. The phosphorylation level of ERK1/2 in Sch-B 1 on 3rd, 7th, 14th and 21st (0.974 +/- 0.169, 0.987 +/- 0.149, 0.920 +/- 0.092 and 0.884 +/- 0.078) and Sch-B 2 on 14th, 21st and 28th (1.012 +/- 0.050, 1.167 +/- 0.083 and 1.002 +/- 0.060) were significantly lower than in silica groups P < 0.05 or P < 0.01); The phosphorylation level of JNK1 in Sch-B 1 reached its summit on 7th day (P < 0.01 ), and that in Sch-B 2 on 14th, 21st and 28th (0.882 +/- 0.064, 0.802 +/- 0.061, 0.792 +/- 0.015) was lower than Silica groups at every time points (P < 0.01); The phosphorylation level of p-p38 in Sch-B 1 was higher than silica group on 3rd day (0.309 +/- 0.045) and lower on 7th, 14th and 21st day, but that in Sch-B 2 were lower and lower. 2. The ratio of nuclear transfer of NF-KB in Sch-B 1 on 3rd, 7th, 14th and 21st [(13.54 +/- 5.36)%, (21.01 +/- 6.43)%, (30.55 +/- 6.44)%, (37.39 +/- 9.32)%] was lower than that in silica groups (P < 0.01); but in Sch-B 2, it was lower than silica group on the 21st [(44.33 +/- 22.88)%] and higher on the 28th day [(58.52 +/- 14.57)%] (P < 0.01).

CONCLUSIONSSch-B has some depression effects on the activation of MAPKs in rat lungs exposed to silica. The nuclear transfer of NF-kappaB could be suppressed by Sch-B in the initial stage of rat lungs exposed to silica. Protection of Sch-B against silica induced lung injury in rats may be via MAPKs and NF-kappaB signal transduction pathway.
